Please allow up to 3 business days for your request to be processed. Once your request is received, the email address will be removed from our medical records system.

In some cases, email addresses may be associated with more than one patient chart.
If this request is for a specific patient chart and you know the health card number please enter it here. (Health card number is optional, if not entered we will remove the email address from all patient charts that contain this address.)

We'd like to keep track of the reasons that our patients choose to unsubcribe from our email service. Please choose a reason from the following options:

I do not recall signing up to receive emails from MAFHT.

I need to change my email address to a different one.

Someone signed up for me, but I did not want to receive emails from MAFHT.

I receive too many emails from MAFHT.

No specific reason, I simply wish to unsubscribe.

I am no longer a patient of MAFHT.

I am no longer the substitute decision maker for the patient in question.

I am no longer the guardian of the patient in question.

I am concerned about my privacy in receiving emails from MAFHT.
