Anyone born in 2016 or earlier is eligible to get the COVID-19 vaccine. Please note that COVID-19 vaccines are not being offered at our clinic. We encourage everyone who is eligible to book an appointment through the Ontario Booking website, or at a participating pharmacy.

Unfortunately, the Montfort Academic Family Health Team is not currently accepting new patients, and does not have a wait list for people wishing to become patients at the clinic. If you do not have a family health care provider and you are looking for one, you can sign up for the Health Care Connect program at the Ontario Ministry of Health and Long-Term Care's website, or by calling 1-800-445-1822.
